Most forms for new and returning students are available on the FACTS Family Portal and can be filled out electronically.  These include the Over-the-Counter Medication Authorization Form, the Epi-pen Authorization Form, the Inhaler Authorization Form, and the Student Driver Form.

The exception is the Commonwealth of Virginia School Entrance Health Form which must be printed out and completed by your child’s physician. This is required of all new students to The Summit Academy and must be submitted to the school administration before the start of the academic year.

All student athletes are required to have an up-to-date physical to participate in their chosen sport. Physicals must be turned in before the first practice, tryout, or conditioning session. Please submit the following physical form only to the front office or the Athletic Director:

Please read the directions on the Teacher Recommendation Request Form carefully. Note that you should give your teacher at least 2-3 weeks to complete your letter, and carefully review each college application deadline before you initiate a request.
